I'm Injured...what are the
steps I need to take? |

- Step 1. Report to your Supervisor that you have been injured.

- Step 2. Fill out and sign the
First
Report of Injury document and submit
the completed form to your supervisor.

- Step 3. Select a physician from the Panel of Physicians. Fill
out and sign the Employee Choice of Physician Form. Be sure to keep a
copy for your records.

- Step 4. Visit your selected physician from the Panel of Physicians.

- Step 6. Notify supervisor of your situation and when the
Physician recommends you return to work.
|
- Step 7. Fill out Workers Compensation Benefits form and attach
it to your timesheet.

- Step 8. If you will miss work due to your injury, your
Supervisor should complete the Employer's Supplemental Report and submit
to the Insurance office.
